I hereby authorize Yoder Enterprises, Inc. (d.b.a./
Advanced / Superior Paging) to change the pay phone status of my 800/888
pager number from pay phone blocked to pay phone accessible, so that I
may receive calls to my pager which originate from pay phones. I understand
that by changing the status of my 800/888 pager number to pay phone accessible
I will be billed $10.00 per month (in addition to regular monthly airtime).
I also understand that I will be billed at the rate of $0.35 (US) (or current
pay phone charge) for each and every call that is placed to my pager from
a pay phone in excess of forty (40) per month. I also understand that
Advanced / Superior Paging has the right to block my pay phone accessible
800/888 pager number if Advanced / Superior Paging deems pay phone usage
to be excessive.
